The Tarrant County Criminal District Attorney’s Office utilizes a special “Worthless Check Information and Affidavit” form that must be completed by the hot check victim when filing checks with the office. By clicking on the link below, you may open and save a copy of this form. The form is in Adobe Acrobat format and requires the use of the free Adobe Acrobat Reader for viewing, saving and printing. Most computers already have this free program installed, however, if you do not a link is provided below to download it.

 
How to use the form:

Three copies of the form are required for each filing (one copy will be returned to you as proof of filing;

The forms must be completed by printing or typing;

The forms must be filled out completely;

They must be fully read, sworn to and signed before a Notary Public, (we can provide that service in our office at no cost);

The forms, with original or substitute check, copy of the notice letter sent, certified or registered receipt, or affidavit of service, invoice if necessary must be included with the filing. If returned, please include the complete returned notice letter sent to the check writer.
